- Engaging families in long-term home visiting services, identifying their strengths, and developing goals to address challenges.
- Competing parent surveys and enrolling families in services when appropriate.
- Meeting with families in the home for at least one hour at each visit.
- Establishing a trusting relationship with families.
- Assisting in strengthening the parent-child relationship.
- Providing strength-based, family-centered interventions to prevent or close child protection cases.
- Assisting parents in improving skills to optimize the home environment.
- Improving the family support system.
- Increasing the family's ability to problem solve and assume the role of advocate for themselves and their children.
- Intervening in crisis situations to provide a safe environment for families.
- Referring, advocating for, and following up with clients in need of additional community support and services.
- Documenting all interactions in the electronic health record, including but not limited to: case notes, assessments, intakes, surveys, discharge summaries, transfer summaries, and evaluations.
- Some evening work may be required.
- You hold a minimum of an associate degree in human services, social work, education, or related field, or demonstrably equivalent knowledge gained through alternative courses of study and life experience.
- You have a minimum of two years of directly related work experience.
- You have some experience working with families in home and/or community settings.
- You are bilingual in Spanish and English, which is required.
- You have experience working with and an understanding of communities living in poverty, communities of color, gay, lesbian, bisexual and transgender individuals, and other vulnerable populations.
- You are able to work respectfully and communicate effectively with different cultures, groups, and languages
- You possess excellent verbal and written communication skills needed to engage families effectively and document the work properly.
- You are able to provide home visiting services and travel to clients' homes, schools, meetings with community partners and various other locations.
- You have access to reliable transportation and are willing to transport clients in your vehicle.
- You can develop and maintain positive and respectful relationships with clients.
- You can track and maintain data necessary for program monitoring and evaluation.
- You can perform assessments needed in order to conduct appropriate intervention and referrals.
- You have a valid driver's license, access to reliable transportation, and the ability to meet TFP's driving record requirements. A Typical Day in the Life...
A typical day as The Family Partnership's Bilingual (Spanish) HFA Family Support Specialist provides an opportunity to assist Spanish and English-speaking families participating in the TFP's HFA Program in identifying their strengths while working to overcome challenges, setting goals, and striving toward positive change in order to prevent child abuse and neglect. The Family Support Specialist will intervene in crises; provide weekly visits, advocate for services to families at risk of or engaged in children protection, and will include the following:
