    Job Description: MHSS and Crisis Stabilization Service

    Report to: Chief Executive Officer Supervision: Clinical Director

    Job Summary: Responsible for offering in-person services to clients, creating service plans, and providing an individual orientation to go over policies and expectations when individuals join the intensive in-home program.

    Qualifications:
    Licensed Mental Health Professional (LMHP) or LMHP Supervisee or Resident
    Certified Pre-screener
    Qualified Mental Health Professional-Adult (QMHP-A)
    Qualified Mental Health Professional-Child (QMHP-C)
    Qualified Mental Health Professional-Eligible (QMHP-E)

    Responsibilities:

    • Assist in providing services according to the treatment plan and updates
    • Model and teach problem-solving techniques
    • Offer various interventions in clients' home and community as needed, such as ADLs, budgeting, health and safety skills, functional skills, effective communication skills, behavioral, social and recreational support
    • Link clients to community-based services and provide advocacy and family support
    • Role-play with clients to schedule appointments and arrange transportation when necessary
    • Provide 24-hour crisis intervention support
    • Ensure proper and timely documentation of all required information
    • Regularly communicate with referral agencies and other involved parties
    • Participate in discharge and aftercare planning with referring agencies
    • Attend interagency meetings as needed
    • Conduct team meetings, orientations, in-service training, and conferences as scheduled
    • Participate in supervision
    • Accompany clients to appointments and within the community
    • Develop and implement Individual Service Plans
    • Complete screening and intake procedures
    • Gather relevant medical and psychiatric history for clinical assessment
    • Refer clients to other professionals if required
    • Undertake additional duties assigned by the administrative department
    • Have knowledge of the Intensive In-home program
    • Attend Human Rights Meetings quarterly and as necessary
    • Engage in client marketing activities until agency growth necessitates hiring a marketing director
    • Work with clients in both home and community settings
    • Coordinate client transportation for medical and scheduled appointments

    Required Knowledge, Skills, and Abilities:

    • Ability to complete First Aid, CPR, and behavior management training (TOVA)
    • Valid driver's license with a good driving record
    • Reliable transportation with liability coverage
    • Demonstrate the ability to work with minimal supervision
    • Strong documentation and communication skills

    Basic computer skills
    Must pass criminal and sex offender background checks
    Provide weekly timesheets (No overtime work permitted by Life Enhancement Services)
