- Develop, oversee, and implement all nursing procedures of the Center as required by regulations and as directed by the Director of the Center and licensing.
- Responsible for the initial assessment of participants to determine the need for both nursing and personal care services prior to the participant's enrollment, dated, and signed by the RN.
- Responsible for the development, implementation, and monitoring of individual care plans for both nursing and personal care. Short-term and long-term nursing goals are specified in each plan of care and shall include monitoring vital signs, baseline measures of vital signs, monitoring weight, nutrition, personal hygiene, and continence.
- Reassessment of both nursing and personal care services dated and signed by the R.N., as well as documentation of the provisions of all skilled nursing and personal care for ADHC participants on the day it is given, either by a narrative notation or a flowsheet that specifies the method of treatment and the reaction of the participant. Must be signed by the person providing the service.
- Interface with primary physicians to facilitate the completion of all forms necessary for participant admission for ADHC. Notify the physician of any marked changes in the participant's condition.
- Oversee that all participants of ADHC/ADC have a tuberculosis test or chest x-ray.
- Interface with ADHC Medical Director and/or PMD in the initial assessment and ongoing treatment of participants.
- Blood pressure, pulse, and weight will be monitored monthly or as needed. Any change in the participant's condition will be noted, recorded, and reported to the family and/or primary care physician.
- Responsible for quarterly progress notes on all participants regarding nursing and personal care services dated and signed by the R.N.
- Reports to the Director any complaint or concern having to do with staff, volunteers, or participants.
- Cooperates with research to implement all aspects of research and evaluation of nursing components of the program.
- Provision of one or more services as determined by the assessments and specified in the plan of care.
- Responsible for monitoring, administering, and record-keeping of medication as specified in the ADHC certification.
- Development of a continence training program if the participant can benefit and oversee license requirements of extra clothing for participants.
- Responsible for the supervision of program aides in conjunction with the implementation of nursing care plans and PT/OT maintenance program.
- Responsible for counseling with participants, families, and/or caregivers to educate and deal with issues pertaining to the participant's health.
- Responsible for providing health education and counseling to the participant and family when indicated by the plan of care.
- Implement emergency procedures in the event of an emergency, including arrangements for transportation.
- Provide supportive and restorative nursing care as indicated in the care plan.
- Provide observation and monitoring of the health status of the participant.
- Provide training in self-administration of medications to participants.
- Meet with the dietician to adjust menus to provide for the special dietary requirements of participants when low-sodium and low-cholesterol diets are provided by the nutrition site.
- Bachelor's Degree in Nursing and licensed by the California Board of Registered Nurses with significant relevant experience.
- CPR/BLS Certificate from American Heart Association.
- Minimum of two years' experience working in the aging field such as Adult Day Health Care, gerontology treatment program, administrative and teaching experience.
- Knowledge and understanding of problems affecting frail elderly and their families.
- Record keeping and assessment experience.
- Bilingual Ability (Spanish/English) preferred.
- Knowledge of word processing, data base, email, Internet, Electronic Health Record, and spreadsheet software/applications.
- Knowledge of and adherence to HIPAA regulations and patient confidentiality.
- Commitment to providing compassionate and culturally sensitive care to underserved populations.

Purpose:
The Community Based Adult Services program seeks a highly experienced and compassionate Registered Nurse (RN) dedicated to serving older adults and individuals with disabilities. As a pivotal member of a multidisciplinary team, the RN collaborates closely with social workers, program aides, physical therapists (PT), occupational therapists (OT), speech therapists (ST), and a Dietitian. Together, they assess and support program participants, striving to keep them in their communities, thus preventing unnecessary hospitalizations and nursing home placements. The RN's role is central to coordinating all health-related activities within the program. This includes determining nursing and personal care requirements for each participant, engaging with primary care providers, caregivers, and offering comprehensive health education to staff, participants, and their families. With their expertise and dedication, the RN ensures that all health needs are met, fostering an environment of well-being, independence, and support for everyone involved in the program.
