Job Description
GHI is seeking an LPN or RN to be a part of our family-oriented team. Pay rate is negotiable. You will be able to work from home and travel to clients within a 30 mins radius Gainesville

Evaluate, assess, and monitor the needs of members via phone, teleconference every and/or f2f visits every 2 weeks
Evaluate and assess CNA, PCA, and HHA as needed
Assist with the assessment of timesheets by signing them weekly
Complete Supervisory Notes
Inform members about any changes made at GHI Homecare regarding operations
Report complaints or other issues
Supervise management of records
Directly reports to the Director.
Ability to communicate effectively
Establish a compassionate environment by providing support to member's family
Be able to provide assessments via EVV app
Ability to complete Nursing assessments as needed
Supervisory visits when necessary
The above duties are not limited to and may include additional responsibilities as needed
