Our Early Intervention Team is committed to leaving everyone better off by providing case management services to children 3 and under diagnosed with developmental delay.
Summary of Responsibilities:
The primary purpose of the Early Intervention Service Coordinator position is to provide service coordination to infants and toddlers with developmental delays and their families through a comprehensive program of integrated services.
Service Coordinators serve as a bridge between families and providers, ensuring that the families' needs are being met, the coaching model is being observed and providers are working within the child's natural environment.
The Service Coordinators ensure that providers have the support and the documentation required to serve the child and family.The Service Coordinators also ensure that there is a plan in place for the family before the child ages out of the Early Intervention program.
Essential Functions:
- SAM Early Intervention is a growing program that provides support to families of children birth to three years of age with disabilities or special developmental needs, helping them to build on their child's strengths, and to enhance and promote the child's growth, development and learning skills in their natural environment.
- Early Intervention Service Coordinators work with children who have disabilities or special developmental needs, assisting them in learning developmental skills such as gross motor, fine motor, and language, and facilitating engagement in activities, routines, and events of everyday life.
- The SAM Early Intervention Service Coordinator will work closely with families and be a liaison between families and service providers.
- Service Coordinators will provide support and services in the context of the family's typical routines and activities so that information is meaningful and directly relevant to supporting the child to fully participate in his or her primary caregiving relationships and environments.
- Process referrals, schedule initial meetings with families, and conduct Screenings using standardized protocol and tools approved by OCDEL and SAM Inc. to ensure timely delivery of services.
- Ensure proper maintenance of child files, required documentation, tracking tools, and other computer programs.
- Coordinate, attend, and schedule assessments; IFSP (quarterly, 6 months, and annual), IEP, eligibility determination meetings, and transition planning; notify all interested parties of meeting dates and times.
- Demonstrate knowledge in accordance with OCDEL Standards and Professionalism, Child Development, Family Professional Partnerships, Teaming, Assessments and Evaluation, Planning, and Intervention.

Education and Experience:
Bachelor's Degree in Early Childhood Education, Psychology, or a related field and 1-year experience working with or providing counseling to children, families or individuals with disabilities.
An associate degree or 60 credit hours in Early Intervention, Early Childhood Special Education, Psychology, or a related field plus 3 years of working full time with or providing counseling with children, families, or individuals with disabilities or Certification by the PA Civil Service Commission meeting the qualification of a Caseworker 2 or 3.
Employment Requirements:
Automobile and Valid Insurance, Valid Pennsylvania Driver's License, acceptable Motor Vehicle Record Check, Act 33 and Act 34 Clearances, FBI Fingerprint Clearance, Social Security Card, Immigration and Naturalization (I-9), Drug Screening Clearance
Working Conditions:
Prolonged periods working on a computer in an open office environment, comfortable communicating through virtual technology, work may be required at nights and on weekends, travel throughout the region, travel may be in adverse weather conditions, must be able to lift up to 15 pounds.
