REMOTE POSITION
Providing suicide prevention case management and crisis intervention services to veterans and their families.

  • Expeditiously screen households for eligibility using a Housing First approach by "screening in" households.
  • Work quickly to complete assessment and develop an individualized service plan with each household.
  • Provide case management services including intake, assessment, financial entitlements/benefits, budgeting, crisis intervention, case conferences, referrals for adjunctive services, advocacy, and liaison with other community and governmental agencies.
  • Provides linkage and referral for mental health, substance use, legal, financial, housing, and other service needs.
  • Meets with participants in community settings such as shelters, jails, hospitals, and private residences.
  • Documents service delivery accurately and timely.
  • Enters intake, contact, individualized service plans, temporary income support assistance, and other information in the electronic health record in a timely and accurate manner.
  • Transports veterans in agency vehicles in a safe, cautious, and responsible manner.

Qualifications:


  • Preferably hold a Master's Degree in Social Work, Criminal Justice, Psychology, Counseling, Mental Health, Human Services, or a related discipline.
  • NJ Licensure in mental health or substance use discipline preferred.
  • Must have crisis intervention and/or suicide prevention experience.
  • Minimum of 2 years of experience in case management, preferably with veterans and a focus on suicide prevention.
  • Must have skill in community resource development.
  • Bilingual (Spanish/English) preferred, but not required.
  • Must possess a valid driver's license; NJ Driver's license in good standing.
