Description

_Seeking a QMA that will be willing to also work as a CNA when needed._

Demonstrates Proficiency in the Following Tasks:

QMA duties:


  • Count, administer and document controlled substances.
  • Empty and change colostomy bag.
  • Conduct diabetic urine testing.
  • Collect fecal or urine specimens.
  • Administer medication through a Gtube or Jtube.
  • Conduct hemoccult testing.
  • Conduct finger stick blood glucose testing.
  • Provide site care and dressing to a healed Gtube or Jtube site.
  • Prepare and administer oral medications, nasal medications, eye medications, ear medications, and rectal and vaginal medications.
  • Instill a commercially prepared, disposable enema.
  • Observe resident's reactions to medications and records same in medical record and treatment record.
  • Follows physicians' orders.
  • Obtains verbal report from previous shift and gives verbal report to following shift.
  • Take vital signs and records same accurately in the medical record.
  • Reports to and request assistance from the charge nurse for condition changes of resident requiring notification of physician and and/or parental medication administration.
CNA duties;

  • Makes occupied and unoccupied beds.
  • Gives tub, bed and partial baths.
  • Gives showers.
  • Provides AM and/or PM Care.
  • Provides skin care and back rubs.
  • Provides nail care.
  • Provides daily hair, makeup and shaving care.
  • Provides proper oral care including the dentures, teeth, gums and mucus membranes.
  • Assisting with bowel and bladder training programs.
  • Use of the Hoyer lift.
  • Obtaining residents' weights.
  • Admission duties.
  • Postmortem care.
  • Catheter care.
  • Accurately measures and records urinary output.
  • Serves, sets up trays and feeds residents as needed.
  • Knowledge of diabetic replacement.
  • Maintains residents privacy at all times.
  • Vital signs.
  • Charting legibly and correctly.
  • Promptly answers resident call lights.
  • Assists residents with ambulation and range of motion exercises.
  • Turns and repositions residents every 2 hours.
  • Dresses or assist in dressing residents in stain free, well repaired, color coordinated clothing.
  • Accurately measures and records oral intake.
Demonstrates Knowledge of Infection Control and Safety Practices

  • Infection Control
  • Universal Precautions.
  • Use of personal protective barriers.
  • Handwashing techniques.
  • Proper handling of linens.
  • Bedpan, urinary and bedside commode cleansing.
  • Cleans environment when needed.
  • Disposes of contaminated wastes as per policy.
  • Cleans bath tub, shower and shower chair after each resident.
  • Proper handling of water to avoid contamination.
Safety

  • Proper use of side rails.
  • Proper use and placement of call lights.
  • Proper use of restraints.
  • Proper use of body mechanics.
Demonstrates the Art of Resident Care

  • Maintains a professional demeanor.
  • Keeps resident rooms straightened.
  • Report changes in residents to charge nurse.
  • Responds to residents tactfully.
  • Utilizes therapeutic communications with confused residents.
Demonstrates Indirect Nursing Care

  • Returns equipment to designated areas.
  • Collects and distributes clean water pitchers.
  • Rinses soiled linen and/or clothing before depositing in the linen hamper.
  • Assists in taking residents to social and activity events.
  • Keeps social services aware of resident needs (i.e., toothpaste, socks, deodorant).
  • Adheres to the routine of the shift.
  • Adheres to the time schedule of responsibilities (dining room, breaks, cover).
Demonstrates Adherence to Personnel Policies

  • Wears appropriate uniform.
  • Mindful of own personal hygiene.
  • Works assigned work schedule.
  • Makes requests for time off in a timely manner.
  • Notifies of inability to work assigned schedule at least 2 hours in advance of shift start time.
  • Provides 2 week written notice to Director of Nursing.
Supervision

  • Is directly responsible to the charge nurse on the unit.

Qualifications

Educational

  • Must be 16 years of age, or high school graduate to emancipate for full time employment.
  • Is able to read and write in English.
  • Current CNA certification.
  • Current QMA certification.
Previous experience is desirable but not required.

Physical:

Shall be in good health without lifting restrictions, and able to meet the requirements set forth by the Indiana State Board of Health.

Conditions of Work and Special Demands

  • Requires lifting, bending, and turning.
  • Is on feet most of the time.
  • Works under physical and mental stress.
  • Is exposed to odors, body secretions and excrements.
  • May be exposed to abusive language and other antisocial behaviors.
  • May be required to provide additional staffing on day off.
