    RN Care Manager:

    In this role, you will serve as a private nurse providing holistic care to your clients. Tasks may range from care coordination & medication management to personal outings & emotional support.

    The RN Care Manager, as part of a care team, will be responsible for the planning and coordination of all services provided to clients and will serve as the clinical expert to all the staff members assigned to their caseload.

    The RN Supervisor will:

    • Provide oversight to all in-home caregivers
    • Perform routine in-home supervisory visits per UT State Regulations
    • Mentor and guide caregivers within their assigned region
    • Provide education to caregivers, staff, clients, and other parties as needed, on clinical issues ranging from disease process to how to appropriately perform hands-on tasks

    Other Duties (include but not limited to)

    • Community Outreach/Support
    • Intake Assessments and Consultations
    • Performance Management
    • Care Management Services

    Knowledge and Skill Requirements:

    • Educational Requirement: Registered Nurse
    • Ability to interface effectively and professionally with client committees, including family, physicians, attorneys, etc.
    • Ability to work in a team environment
    • Excellent written and verbal communication skills required.
    • Valid driver's license and proof of current auto insurance required.
    • PC literacy required.

    Knowledge and Skills Preferred

    • A minimum of two (2) years' experience in Nursing
    • Ability to interface effectively and professionally with client committees, including family, physicians, attorneys, etc.
