Tagged: Shift - Day, Full TimeDUTIES/RESPONSIBILITIES:
Responsible for all aspects of assigned caseload, including meeting with individuals and families living in supportive housing on a regular basis. Make referrals to medical, mental health and/or substance abuse services as needed. Provide clients with supportive counseling. Make regular visits to client apartments, and monitor clients to ensure a safe and secure environment. Teach and assist clients with activities of daily living. Plan and facilitate support groups. Assist with entitlements. Ensure that all clinical files and reports are complete and up to date. Assist with intake and admission process. Participate in community meetings. Provide emergency first aid/CPR assistance when needed. Related duties as assigned.
HOURS :
Full-time 37.5 hours per week
QUALIFICATIONS:
BA preferred. At least four years relevant experience and HS diploma/ GED required. Experience working with the substance abuse and/or mentally ill population required. Bilingual in Spanish required. Basic computer literacy required. CPR training certification or willingness to take training class in CPR.

PROGRAM DESCRIPTION
Liberty Homes is a permanent supported housing facility with 46 units funded by the NYC Department of Housing Preservation and Development and the US Department of Housing and Urban Development. The program serves disabled and formerly homeless single adults, couples and families. In addition to providing permanent housing for its tenants, Liberty Homes offers a variety of services aimed at helping clients maintain their independence and build a community of support among peers, including case management, referrals to medical facilities, health education, psychiatric management, employment search and retention assistance, parenting training, entitlement support and money management, recreational activities, and assistance with activities of daily living.
