Mission:


The LPN/HM is responsible for providing professional and quality person centered care for residents with Alzheimer's disease and other types of Dementia to promote their highest well-being.

The LPN/HM is also responsible for ensuring compliance with company policies and state regulations. The LPN/HM reports to the Regional Director of Clinical Services.

Job Duties and Responsibilities:


Clinical:


  • Practice a holistic approach to meet residents' needs and to promote their highest wellbeing.
  • Evaluate residents prior to admission
  • Visit and reevaluate current residents who may be in a temporary acute care stay
  • Utilize nursing process by demonstrating appropriate assessment, planning, implementation, and evaluation of the residents' care, as evidenced by staff observation.
  • Collaborate with Executive Director and Director of Clinical Services and team members to ensure the residents' wellbeing and interdisciplinary approach to care.
  • Oversees medication process and administration.
  • Oversees infection control measures for the home

Administrative:


  • Uphold operational policies, procedures and systems
  • Understand and maintain current CBRF and State regulations
  • Assist Executive Director with filling in holes in the schedule as needed
  • Monitoring of electronic chart in all aspects
  • Routine and PRN assessments
  • Participates and help lead monthly team inservices and ongoing education
  • Assists with scheduling as needed

Assessment and Care Planning:


  • Admit and discharge residents according to admissions criteria
  • Complete individual service plan for each resident with interdisciplinary team and ensure service delivery according to individual residents' needs
  • Monitor the health and emotional conditions of each resident
  • Perform documentation and observation techniques to benefit residents
  • Ensure safe and appropriate administration of medications by properly trained staff

Family and Support:


  • Actively participates with problemcomplaint resolution to attain satisfaction for the residents and families

Personnel Management:


  • Lead by example, encourage teamwork, promote the MOSAIC philosophy and provide ongoing coaching to employees
  • Respond to onthejob injuries in accordance with policies and procedures
  • Perform coaching and disciplinary discussions with staff as needed
  • Maintain the established staffing model and stafftoresident ratios
  • Assist the ED with interviewing and hiring process
  • Train new staff on the floor

Education:


  • Current, unencumbered active license to practice as a Licensed Practical Nurse in the State of Wisconsin
  • One year of longterm care experience preferred
