To be considered for this position, you must have Quality Assurance experience in a Residential Health Care Setting.

Summary/Objective


The Quality Assurance Coordinator (QAC) is responsible for overseeing and assuring that Quality Assurance within the residential homes is reflective of guidelines and standards.

The position is a neutral position which should ensure the health and safety of the individuals and the agency.

QAC should ensure agency personnel/systems are working in accordance with Above Expectations/DDDS Policies and Procedures, HCBS Standards, and DHCQ standards and regulations.


Essential Functions

  • Possess a comprehensive understanding of DDDS HCBS Standards, Policies and Procedures along with DHCQ Standards.
  • Relay reported incidents to Operations Leadership and Human Resources
  • Reviews and investigates reported incidents in coordination with COO and PC
  • Collaborate with the Operations Leadership team to formulate remediation plans as part of QIP process
  • Ensures proper communication(s) of reportable/nonreportable incidents
  • With cooperation of COO and PC, work to ensure DDDS is aware of incidents which impact the health, safety and wellbeing of the service recipient(s) in accordance with PM#46/DDDS policy, reporting expectations, and PM#
  • Identify and assist with developing corrective actions to prevent or reduce the likelihood of future similar incidents
  • Evaluate interventions to ensure remediation strategy was successful
  • Auditing of internal QA Therap system and ensuring all homes meet and maintain all standards
  • Reviews and follows up on Tlogs, GERs and ISPs as necessary
  • Daily audit of Therap and follow up on missing documentation (ISP, GER, Tlogs etc)
  • QA will complete and submit documentation audits in full for all homes on the 14 and 28 of each month to COO, PC and responsible PS
  • Attend all nursing audits and ensure PS and staff follow all recommendations by the consultative nurse
  • Identify opportunities for improvement that exist throughout the program and share with Operations Leadership
  • Reviews PSRs weekly and determines any financial discrepancies
  • Demonstrates extensive knowledge of the Therap documentation data base specific to EMARs, tracking appointments, current medications, and medical history
  • Coordinate medication with Tarrytown pharmacy and notify PS's when to pick up medications
  • Follow up on medication changes noted on MAIR/PAIR/CAIR/DAIR
  • Ensure all follow up appointments noted on MAIR/PAIR/CAIR/DAIR have been entered into Therap accurately
  • Ensures all PRN medications have appropriate Therap documentation and follow up as required by LLAM protocol
  • Ensures all appointments are in Therap with correct attachments and marked as completed
  • Reviews all individuals general event reports (GERs) to ensure completion and accuracy and notification to all required parties and report discrepancies to COO and PC
  • Biweekly visits to all homes to ensure all DDDS standards are maintained
  • Attends and participates in TPC meetings
  • Ensure all required prerequisites are received and completed prior to new individual move in (when identified)
  • Assist in random and annual DDDS and DHCQ State Quality Service Reviews
  • Collaborate with the operations leadership team to respond in WellSky and complete quality improvement plans which consist of the following
  • Provide a summary and update of open and newly closed QIPS to COO and PC during QIP meetings
  • Collaborates with COO and PC for policy and procedure creation and review

Requirements:


  • Must have experience in Residential Health Care Setting
  • Minimum 3 years QA experience

Pay:
$50, $70,000.00 per year
