Seeking a compassionate and dedicated Hospice Nurse to join our team in Beaumont, TX.As a Hospice Licensed Vocational/LPN Nurse, you will play a crucial role in caring for our patients in their homes.
Under the supervision of our Registered Nurses (RNs) and in accordance with physician orders, you will ensure our patients receive the highest standard of nursing care and support.
This position offers flexible scheduling options, whether you are looking for PRN, part-time, or full-time work. We offer pay visit and/or hourly compensation.PRN Per Visit Rate:
$60 per visit. Part-time or Full-time hourly pay between $24 to $28.
Key Responsibilities:
- Provide direct patient care as prescribed by the physician and in compliance with the policies of the agency.
- Assist in the development and implementation of the Plan of Care.
- Monitor patient health condition, record signs of progress or decline, and communicate findings to the supervising RN.
- Administer medications and treatments as prescribed by the health care provider.
- Educate and provide compassionate support to patients and their families about endoflife care.
- Maintain accurate and timely documentation of all care provided.
Qualifications:
- Valid Licensed Practical Nurse/LVN license in the state of Texas
- Minimum 1 year of experience as a LVN/LPN in hospice care is highly preferred
- Reliable transportation and auto insurance
- Experience with electronic health record systems (e.g., Hospice Kinnser)
- Excellent communication skills, both verbal and written
- Ability to work collaboratively within a multidisciplinary team
