Position Description:

Penn Medicine at Home is seeking a per diem Nurse Practitioner to join their growing team The Nurse Practitioner will be cross-trained to provide Palliative Care and Primary Care services to adult patients in the home setting seeing a combination of palliative care consults in the home, as well as primary and transitional care.

Shift opportunities exist Monday through Friday from ~8:30am to 5:00pm across Philadelphia, Delaware County, Montgomery County, Chester County, and Bucks County.
