- Perform patient assessments and develop individualized care plans

- Monitor patient vital signs and report any changes to the healthcare team
- Provide patient education on health maintenance and disease prevention
- Document patient care activities in electronic health records (Epic)
- Adhere to HIPAA regulations to maintain patient confidentialityQualifications:
- Must be a Licensed Practical Nurse (LPN) in good standing
- Proficiency in medication administration and dosage calculations
- Experience in senior care, family planning, and patient assessment
- Ability to perform basic life support measures in emergency situations
- Familiarity with electronic health records systems like Epic
