- The Habilitation Specialist 1 meets with and evaluates individuals diagnosed with Intellectual Disability generates effective habilitative program plans concurrent with their interest and needs.
- Trains direct support staff on implementation of habilitation outcomes.
- Trains direct support staff on the documentation of the habilitation outcomes.
- Monitors program planning outcomes by observation, discussion with direct support staff.
- Maintain records for all assigned cases according to the facility standard, maintains Care Coordinator and advocate contacts, facilitates meeting with residential team and care coordinator.
- Liaises with staff in the Financial Benefits and Entitlements Management Office for related matters.
- Generates all documents in accordance with Home and Community based Waiver Services.
- Practices all standards relevant to Medicaid Program relating to HCBS delivery.
