    JOB TITLE: Substance Misuse Prevention Coordinator

    DEPARTMENT: Strafford County Public Health Network

    POSITION STATUS: Full-time, hourly - Non-Exempt

    GENERAL JOB DESCRIPTIONS: The Prevention Coordinator leads our regional efforts related to substance misuse prevention and related topics by coordinating health promotion activities and strategies to implement, promote, and advance evidence-based primary prevention approaches, programs, policies, and services. They work as part of our SCPHN team on strategies, programs and projects related to substance misuse, mental health and social determinants of health as they intersect with other roles and initiatives. They utilize a public health approach using the Strategic Prevention Framework (SPF) as a guide to prevent and reduce substance misuse risk factors and strengthen protective factors. They support and advance related work within the region by representing the SCPHN program in support of related efforts across our communities. Through community engagement and mobilization, including the facilitation of our Prevention Leadership Group, our Prevention Coordinator acts as a leader in our community and collaborates across our RPHN team and networks to strengthen the systems and infrastructure in Strafford County that support our work in prevention and related topics.

    QUALIFICATIONS

    Bachelor's degree preferred in public health, social work, or related field; or three-five years' experience in public health, community health education, or community organization; Certified Prevention Specialist (CPS) certification preferred, or will be pursued within one year of hire

    DUTIES AND RESPONSIBILTIES

    Accountability

    Measure

    Develop and maintain a regional network of subject matter experts to support substance misuse prevention and related health promotion activities through the facilitation of a Regional Leadership Team

    Regional network of stakeholders is grown and maintained in the Prevention Leadership Group

    Strategic plan is up-to-date & evidently utilizes the SPF in its structure & content

    Progress on the strategic plan is demonstrated & documented in reporting required or requested by DHHS, GSCH or SCPHN leadership

    Coordinate activities within various work plans such as those requested by DHHS, SCPHN's current CHIP, others related to other funded initiatives/programs, or otherwise

    Work plans are up-to-date

    Progress on work plan activities are demonstrated & documented in reporting required or requested by DHHS, GSCH or SCPHN leadership

    Ensure that programs and priorities are data-driven, evidence-based, and culturally appropriate to achieve outcomes;

    Data is collected & used as a planning & evaluation tools for all projects & programs

    Submit regular reporting as requested and required by NH DHHS, other funders and the organization

    Reporting is complete and up-to-date

    Collaborate with other regional prevention specialists programs, state agencies

    Create & maintain seats at various tables in the community & help facilitate collaboration, communication & shared goals, etc.

    Attend all state required trainings, workshops, and bi-monthly meetings

    SCPHN is represented at statewide & regional meetings, as requested

    Provide strategic planning support for program improvements.

    Sharing data, information and other resources with partners and SCPHN team to support strategic planning efforts; plays an active role in strategic planning efforts relevant to role

    Provide logistics support, program implementation, trainings, workshops and meetings

    Provides technical assistance to partners in the region, hosts community education, training & professional development as appropriate within current priorities and plans

    Develop key social marketing and collateral materials for prevention messaging and building audiences across sectors of the community and the age spectrum

    Integrates messaging via social media and other communication channels into work; works with SCPHN team to maintain an up-to-date website; utilizes platforms like Canva to develop and maintain materials and information relevant to current projects, programs and initiatives

    Physical Demands:
    1.     7. Ability to exert up to twenty (20) pounds of force occasionally and/or a negligible amount of force frequently or constantly to lift, carry, push, pull or otherwise more objects. Essential functions involve sitting most of the time, but walking, standing, reaching and bending may be required occasionally in a typical office setting.
    8. Ability to transport self to meetings and alternative sites when requested.
