Registered Nurse (RN) Case Manager provides medical oversight to ensure that Myland Homecare serves its clients effectively and safely in the community.

Medical oversight includes assessing and monitoring the client's condition and implementing/arranging interventions to prevent or delay unnecessary and more costly institutional placement.

The Myland Case Manager will assign certain tasks to unlicensed and licensed staff.

The Case Manager utilizes the "RN Assignment Decision Tree," generated by the Georgia Board of Nursing, to assist in evaluating the client care tasks on an individual client basis; it guides the Case Manager in assigning only those tasks that can be safely performed by trained unlicensed and licensed assistive staff.

The major duties of the Myland Homecare Registered Nurse (RN) Case Manager include, but are not limited to:

  • Completing all initial new client comprehensive nursing assessment and care plan development.
  • Assessing and evaluating the client's needs, current status, environment, and changes every supervisory visit or more often if indicated by the client's condition.
  • Reviewing the Level of Care for all clients.
  • Reviewing and following up on all LPN supervisory visits and executing the reevaluations of client care within (10) days of each supervisory visit.
  • Nursing staff are prohibited from administering medications to clients or providing any other client care while conducting supervisory visits for all services except skilled nursing. For skilled nursing, the private home care provider's RN can perform a nursing visit and update the care plan as part of the same visit.
  • Developing, coordinating, and revising client care plans. Communicating all revisions to appropriate staff using the staff communication systems.
  • Preparing progress/clinical notes, reviewing progress note entries of all care staff, reviewing and cosigning documentation of all LPN supervisory visits, and instructing staff on charting protocol. The Case Manager will review LPN supervisory visits, as well as the followup and resolution of problems.
  • Conducting and maintaining ongoing communication with other service providers, the physician, and other relevant parties to changes in the client's medical condition or status that require followup and/or additional services.
