The registered nurse plans, organizes, and directs hospice care, and is experienced in nursing, with the knowledge and skill to direct hospice services to meet the needs of the patient and their family.
Job Duties
Completes an initial, comprehensive, and ongoing assessment of a patient and family to determine hospice needs.
Provides a complete physical assessment.
Writing and initiating the plan of care and communicating this to the appropriate members of the interdisciplinary team.
Regularly re-evaluating patient and family/caregiver needs
Develops a care plan that establishes goals, based in nursing diagnosis, and incorporates palliative nursing actions that also includes the patient and family in this process.
Counsels the patient and family in meeting nursing and related needs.
Conducts an on-site visit to the patient's home no less frequently than every 14 days to assess the quality of care and services provided by the hospice team to make sure they meet the needs of the patient.
Perform on-call responsibilities and provide on-call services to patients and families as assigned and requested.
Perform all duties with excellence and diligence, have a great ethical compass and always maintain confidentiality of patient/family. Demonstrate commitment, professional growth, and competency.
Promote the Agency philosophy and policies and procedures.
Qualifications/Requirements
Graduate of an accredited school of nursing with a current and in good standing nursing license in the state of Arizona.
Must have a current driver's license, reliable transportation with the ability to travel to make patient visits.
Current CPR/First Aide Certification
Computer skills; prior experience with electronic medical records (EMR) is preferred, but not required.
Ability to work independently and manage time effectively.
Ensures protection of confidential information that is stored, handled, or maintained by the Agency per HIPAA regulations.
Excellence in observation, verbal and written communication skills, problem solving skills, basic math skills, and nursing skills per competency checklist.
