- Conducts ongoing assessment of client needs, strengths, and resources in order to create housing stability plan and provide appropriate services. Ensures clients have an active voice in their goal/service plans.
- Maintains a caseload of 20-35 clients and provides intensive support services. Support services include: mental health and medical referrals, education and advocacy; benefits advocacy, vocational development, money management; substance abuse treatment support; social services referrals, assistance with procuring housing, coordinating with client's medical health home.
- Recruits landlords to accept subsidized housing clients, works closely with housing authority to secure leases, and maintains on-going relationship with landlords to support client housing stability.
- Coordinates closely with benefits advocacy staff to facilitate SSI application process.
- Participates in regular case conferences, team meetings, and acts as an advocate for participant as issues arise in housing.
- Assists with timely and accurate program data collection.
- Utilizes a Critical Time Intervention Model focus on linking participants to support services and support people in the community.
- Utilizes motivational interviewing techniques to explore participants' substance dependency, encourage reduction and/or provide support to clients moving towards abstinence.
- Develops and maintains working relationships with outside social service agencies, ensuring that clients and staff have maximum access to community resources.
- Identifies and collaborates with support people and family members in participant's life.
- Performs other duties as assigned by supervisor.
- Strong organizational, administrative and problem-solving skills, and ability to be flexible and adaptive to change.
- Excellent interpersonal, verbal, and written communication skills and ability to effectively work with diverse colleagues including landlords, property managers, other employees, community partners and vendors.
- Ability to seek direction/approval from manager on essential matters, yet work independently with little onsite supervision, using professional judgment and diplomacy.
- Work in a team-oriented environment with a number of professionals with different work styles and support needs.
- Lived experience with mental illness, homelessness, and/or co-occurring disorders or experiences as a family member of someone with lived experience preferred.
- Must be able to handle a rigorous case load while meeting deadlines for billing, progress notes, and treatment plans.
- Reliable transportation and proof of a valid and current California Driver's License and current insurance along with a clean DMV record required.
- Conduct oneself in external settings in a way that reflects positively on LifeLong Medical Care as an organization of professional, confident and sensitive staff.
- Ability to see how one's work intersects with that of other departments of LifeLong Medical Care and that of other partner organizations.
- Be creative and mature with a "can do", proactive attitude and an ability to continuously "scan" the environment, identifying and taking advantage of opportunities for improvement.
- BA in social work, psychology or related field or 3 years relevant experience preferred; high school diploma or GED required.
- Knowledge and ability to implement the following evidence-based models; Psychosocial Rehabilitation, Harm Reduction, Housing First, and Motivational Interviewing.
- Ability to build supportive and respectful working relationships with participants and sensitivity to and understanding of the special needs of the homeless.
- 2 years conducting outreach and engagement activities.
- 2 years providing case management/social services/outreach including harm reduction and motivational interviewing techniques.
- Knowledge of psychopathology and treatment modalities for dual diagnosed clients and the ability to assess clients' bio-psychosocial functioning.
- Proficient in Microsoft office.
- Bilingual in Spanish or other language (Mandarin, Cantonese, Vietnamese, Hindi) preferred.

Overview
We are looking for Case Managers at our Supportive Housing Program in Oakland. The Alameda County Coordinated Entry System (CES) is a standardized process for connecting people who are homeless to the resources available based on need. To maintain continuity of care, Housing Navigation (HN) and Tenancy Sustaining Services (TSS) have been organized so that a support staff person may outreach and engage a houseless client, support them in finding housing, transitioning to being housed, and stabilizing in that housing. Major responsibilities include outreach, assessment, identifying housing and working with landlords and providing intensive supportive services to an active client caseload. Services are offered using housing-first evidence-based practices focusing on the core values of health, home, purpose, and community. In addition, Case Managers will work closely with regional health care partners linked with these clients to meet their ongoing health care needs.
This is a grant funded, full time, benefit eligible opportunity.
This position is represented by SEIU-UHW. Salaries and benefits are set by a collective bargaining agreement (CBA), and an employee in this position must remain a member in good standing of SEIU-UHW, as defined in the CBA.
