    About Us - Social Services


    • The Social Services team provides discharge planning to every patient who needs assistance with placements, equipment, services, social issues, etc
    • Each inpatient department is assigned a Social Worker. This includes Freeman West, East and Neosho
    • Our team collaborates with physicians, nurses, ancillary staff, patients, and their families to provide the best care possible
    What You'll Do


    To provide Social Service and Case Management to the Emergency Department population who present with social and/or discharge needs that may prevent them from returning to their current environment.

    Screen and assess suicide risks of assigned patients and maintain clear communication with ED staff and Psychiatry team.

    Will provide hospital-wide Social Service coverage as needed and time allows.

    Requirements

    Social Work, Psychology, Criminal Justice or like degree
