- Provide skilled nursing care to patients in their homes.
- Administer medications and treatments according to the physician's orders.
- Observe, assess, and document patients' condition and notify the appropriate healthcare professionals of any significant changes.
- Develop and implement a care plan in collaboration with the healthcare team.
- Educate patients and their families on disease management, self-care, and medication administration.
- Monitor and evaluate the effectiveness of the care provided.
- Ensure compliance with regulatory standards and policies.
- Maintain accurate and complete medical records.
- Collaborate with other healthcare professionals to coordinate services and facilitate the smooth transition of care.
- Provide emotional support and compassionate care to patients and their families.
- Must be a licensed practical nurse (LPN) or licensed vocational nurse (LVN) with a valid license in the state of practice.
- Graduation from an accredited practical nursing program.
- Prior experience in home health or a related field is preferred.
- Excellent clinical and critical thinking skills.
- Strong communication and interpersonal skills.
- Ability to work independently and manage time effectively.
- Knowledge of medical terminology and patient care procedures.
- Basic computer skills for documentation and communication purposes.
- Physical stamina and the ability to lift and move patients when necessary.
- Valid driver's license and reliable transportation as travel to patients' homes may be required.
