Principle Functions:

The Case Manager (also referred to as Client Care Coordinator) is responsible general case management and coordination of client care services in the clinic and community.

Primary duties include performing needs assessments, creating Client Care Plans and tracking progress, performing home/hospital visits and coordinating client support services (support groups/mentoring program, post-birth care).


Qualifications:

  • Must possess one or more of the following qualifications:
  • Bachelor's degree in related social services field, required.
  • Master's Degree in Social Work with Texas Licensurepreferred
  • Registered Nurse (RN) preferred
  • Certified community health worker
  • Minimum 1 year of case management experience
  • Minimum 1 year of successful care coordination, program assistance, or client coaching experience
  • Special training/certification in motivational interviewing, coaching, mentoring, therapeutic relationships, adult or teen education
  • Able to use database management systems.
  • Exhibit excellent organization skills.
  • Be selfmotivated, dependable and responsible.
  • Work well in a fastpaced environment.
  • Excellent written and verbal communication skills.
  • Demonstrate compassion, cheerfulness, and professionalism.
  • Willing to pursue ongoing training.
  • Willing and able to travel, as necessary
Essential Job Functions

  • Assess the physical, mental, emotional, social, educational, and financial needs of each client and help connect them with services (internal and external) to meet the identified needs.
  • Foster caring and supporting relationships that include family group decisionmaking efforts that promote childbirth;
  • Encourage individuals to reach their fullest potential by improving coping strategies and life skills;
  • Provide guidance and assistance to Clients to develop vision for the future; and
  • Create a Client Care Plan based on needs assessment results and include specific goals the client would like to work towards. Track and document goal acquisition and support clients in transitioning through the steps of their Client Care Plan.
  • Maintain complete and accurate records of all clients using SalesForce database.
  • Provide crisis prevention and management support when needed.
  • Complete required training
  • Perform monthly Due Date follow ups and make home/hospital visits to check in on moms and new babies.
  • Collect measurements and data metrics for different initiatives; report biweekly to Clinic Director.
  • Capture referral calls and minutes for grant reimbursement.
  • Communicate to the Clinic Director any significant barriers clients face and collaborate to find solutions.
  • Provide comprehensive, integrated, and timely referral to government assistance programs and/or county support networks that provide assistance to families before and after childbirth and promote the child's safety and wellbeing.
  • Assist anyone seeking emergency support (diapers, clothing, baby items, etc.)
  • Manage client support groups.
General

  • Maintain strict confidentiality and professionalism when handling sensitive client information
  • Attend weekly team meeting and monthly all staff meetings
  • Assist with special events, including annual fundraisers, 101's and church presentations as needed
  • Establish positive working relationships with staff members and volunteers
  • Other duties, as needed
