    Description
    Community Options, Inc.* is a national non-profit agency providing services to individuals with disabilities in 12 states.
    We are actively seeking *Community Integration Counselors* in *Pottsville, PA*.

    This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.

    No experience required– we provide PAID training**New hires are eligible for a sign-on bonus of $250.50/per hour _
    *Meet with waiver participants on a regular basis either in the individual's residence, the community, or an office setting

    • Provide ongoing support for community integration efforts
    • Document individual's progress toward acquiring community integration
    • Assist in building a support network between the individual and the community
    • Ensure medical consultation forms are completed, by the physician, for each medical appointment attended
    • Monitor the health and medical needs of individuals and immediately report any concerns
    • Drive individuals to medical appointments and activities in the community
    • Bachelor's Degree preferred
    • Minimum 2 years of relevant experience supporting individuals with intellectual and developmental disabilities
    • Transport individuals utilizing your own vehicle or company provided vehicles
    • Schedule may change due to business needs and may include evening and weekend hours
    • May be required to be on-call in cooperation with other management staff
    • May be exposed to various medical conditions and communicable diseases
