Description

The Case Manager is responsible for assisting the providers with case management support by identifying needs and providing resources to patients, as well as coordinating and collaborating with CPR clinicians, health plans, medical/behavioral health providers, and other community organizations.

Outreach, engagement, and coordination will be an important and necessary function of this position, and will be based on individual needs.


Knowledge and Skills:


  • Communication Skills: Must be comfortable engaging in conversation over the phone, and in person, with both patients and other medical/behavioral health professionals, and community organizations. Communication must be professional, calm, and sensitive at all times.
  • Behavioral Health Skills: must possess a working knowledge of behavioral health symptoms, presentations, diagnoses and treatments. Must be able to interact with patients who are experiencing chronic or acute mental illness. This may include deescalation and rapport building in order to obtain needed information.
  • Organizational Skills: Must be able to accurately collect, document, and track a large amount of patient data.
  • Computer Skills: Must be able to accurately and thoroughly document in patient charts and other platforms.

Networking Skills:
Must be comfortable researching, reaching out to resources and community partners to build and maintain a functional referral source base


Duties:


  • Coordinate with various stakeholders to support needs of patients and appropriate levels of care.
  • Engage with patients to support postdischarge from inpatient facilities; provide face to face, telehealth and/or telephonic services.
  • Triage patients, assessing safety and resources needed.
Outreach and engagement, including but not limited to, linking patients to community behavioral health providers, complete skills training as needed, and assist with resources and referrals. Community based outreach and engagement is an important and necessary function of this position, and will be based on individual and program needs May facilitate groups based on program and location Provide extra support to clinicians or stakeholders, when applicable, to patients with acute needs. Coordinate with the patients' treatment team to support further care needs. Assist patients in connecting to a primary care provider and act as a liaison between PCP and behavioral health provider(s) to promote integrated care. Assist patients with referrals, phone calls, paperwork, etc. as needed. Outreach patients to support continuity of care as needed. Complete initial and ongoing assessment screenings for Social Determinants of Health (SDOH) and Health (HRA), when applicable. Other duties as assigned
