Description

CLM provides a wide range of mental health services for children and families. The Case Management Benefits Specialist is responsible for offering outpatient mental health services, including assessment, treatment planning, benefit coordination, and community-based support. This is a full-time position, with 40 hours per week.

Responsibilities:

  • Provide education on benefit resources such as APTD, WIC Nutrition Program, Social Security, and Medicaid;
  • Help clients set up NH Easy accounts;
  • Support clients and staff with NHEASY CLM access for managing benefits online;
  • Assist clients in managing benefits, reporting changes, and accessing work incentives;
  • Collaborate with the treatment team, community partners, and clients for wraparound care;
  • Attend consultation meetings, committees, and ensure proper spenddown management;
  • Coordinate verification collection and maintain client confidentiality;
  • Execute required services and maintain documentation according to standards;
  • Uphold professional standards, memberships, licenses, and certifications;
  • Represent CLM professionally in all interactions;
  • Perform additional duties as assigned.

Qualifications:

  • Bachelor's degree preferred, with 2 years of direct care experience;
  • Proficient in electronic health records and effective communication skills;
  • Ability to work in a team, self-directed, and multitask.

Physical Demands and Working Conditions:

The work is primarily office-based or remote, with meetings via TEAMS. Manual dexterity and sitting for extended periods are required. Flexibility in schedule and reliable internet access and transportation are necessary.
