Personal Support Professional:

HDC is seeking a caring direct care professional in a personal support role. This position is Part-Time (less than 29 hrs a week) transporting and accompanying disabled individuals into the community setting.


Schedule Requirements:

• Evenings and weekend shifts are mandatory and on an asneeded basis.
• Split shifts may be required depending on scheduling demands.

Job Duties:


In the community:

• Facilitate the development of community contacts and social skills, on a 1:1 basis
• Assist in individuals' reaching their outcomes/goals
• Support the individual's needs, sports interests, hobbies, and personal interests, and assist in developing social skills within the community.
• Assist with other duties as assigned

Minimum Requirements:

• Minimum age of 1
• High School Graduate or equivalent
• Possess a valid driver's license with less than four (4) points
• Must clear a criminal background investigation
• Physically lift 50 pounds and carry for short distances
• Be able to stand for long periods without mechanical support
• Provide supervision
