Bear Hill Nursing and Rehabilitation Center located in Stoneham MA is currently looking for a
Director of Social Services with experience to join our team of healthcare professionals.
Responsibilities include but are not limited to:
- Develop preliminary/comprehensive assessment for each resident.
- Report any problems to the Director of Nursing Services/Administrator
- Encourage the resident/family to participate in the development and review of resident's plan of care.
- Develop a written plan of care for each resident that identifies the social problems as well as the needs of the resident and set goals to be accomplished for each problem/need identified.
Qualifications:
- LSW / LCSW with a current social work license in the state of MA
- Experience in LTC preferred
- Excellent written and communication skills
Experience:
- Long term care: 1 year (preferred)
- Social work experience: 1 year (preferred)
Schedule:
- Monday to Friday
Pay:
$80, $90,000.00 per year
