Sun River Health is currently seeking a full-time RN Clinical Manager of Health Home Care Management to join our team in the Bronx, NY at our Townsend site with travel as needed to our Manhattan Office (248 W 35th Street).

**$1500 Sign On Bonus

  • Terms and Conditions Apply**Responsible for developing and coordinating care management activities, workflows and training across all health home sites in accordance with evidencebased practice and/or regulatory/payer requirements to achieve clinical outcomes. Provides ongoing consultation and assistance to care managers on managing difficult/clinically complex cases. Will be working directly with care managers and their high risk clients on their caseloads and offering clinical guidance on how to move them forward. Will also train care managers on best practices and workflows in different areas of care management.

ESSENTIAL FUNCTIONS:


  • Develops and implements use of educational materials and ongoing training plans to ensure staff have the necessary clinical information and resources needed to meet clinical objectives.
  • Works collaboratively with management team to conduct clinical operations analysis, prepare reports and provide recommendations for performance improvement.
  • Abides by and promotes agency administrative policies
  • Participates in current process review and development of new and/or revised clinical workflows, tools, processes, policies and procedures relating to complex care management responsibilities
  • Participates in Agency evaluation program as part of CQI team
  • Maintains current knowledge of policies and procedures, state and local laws, industry best practice standards by participating in formal education programs, conference, workshops and professional organizations
  • Ensures staff receives appropriate orientation and training to care management, care planning and care coordination workflows, best practices and policies and procedures to ensure that staff have the clinical expertise to perform assigned duties.
  • Monitors and evaluates client care plans and services for quality and appropriateness through direct observation, documentation review, and conferences with staff
  • Collaborates with supervisors to guide and monitor staff placed on performance improvement plans for clinical workflows and documentation.
  • Monitors care manager assignments and makes recommendations for adjustments based on staff experience, severity of cases and type of intervention(s) required.
  • Leads team case conferences and coordinates interdisciplinary care planning
  • Participates in on Call assignments and weekend scheduling

EDUCATION/EXPERIENCE:


  • Graduate of an accredited fouryear nursing program
  • Registered Nurse with current New York State license
  • Bachelor's Degree in Nursing required, Masters preferred
  • CPR/BLS required through American Heart Association

$80, $100,000.00 per year
