The Documentation Specialist is responsible to assist the Qualified Intellectual Disabilities Professional at the Day Program with observation, writing, correction, signing, and submitting of monthly case notes for billing purposes.

This position reports to Qualified Intellectual Disabilities Professional (QIDP) and is required to support our Day Programs in Bronx/Yonkers.


Qualifications:


  • Bachelor's Degree Preferred but Associate Degree is accepted in one of the following professional categories: Human Services Professional field (including, but not limited to sociology, special education, rehabilitation, counseling, or psychology.)
  • One year of experience working directly with people with intellectual disabilities or individuals with physical and/or developmental disabilities and comorbid psychiatric diagnoses.
  • Proficiency in Microsoft Office and computer savvy.
  • General knowledge of theories and methods of the behavioral analysis process, teaching strategies, assessments, and goal development.
  • Excellent written and verbal skills.
  • Must be able to travel to programs in Yonkers and Bronx as needed.

Summary of Primary Responsibilities and Essential Functions of Position:

  • Write, proofread and sign monthly case notes.
  • Read, review notified DSPs of corrections, and sign monthly case notes before the due date.
  • Attend and observe the activities of individuals in community outings to ensure the accuracy of the monthly case notes.
  • Ensure the quality and accuracy of the case notes before signing off.
  • Maintain effective communication with the Coordinators & Supervisors at the Day Hab Programs to ensure Direct Support Professionals (DSP) completed, submitted, corrected, and signed monthly case notes in accordance with required time frames.
  • Collaborate with the Quality Assurance (QA) department regarding monthly case notes.
  • Back up coverage and support to the QIDP department in the absence of colleagues.
  • Orientation training for all employees at the time of hire and during their tenure.
  • Attend all departmental and agency meetings/training.
  • Perform all other duties as required by the QIDP and agency.

Clean Drivers License.
