Hospice On-call Nurse -
(7 days on/7 days off)

Job Summary - Hospice:


Primary functions are to provide nursing care for clients of all ages in their place of residence, coordinate care with the interdisciplinary team, patient/family, and referring agency; and assume the responsibility for coordination of care.


Essential Functions - Hospice:


  • Initial and ongoing comprehensive assessments of the impact of the terminal disease on the patient's physical, functional, psychosocial, and environmental needs and ADLs, i.e., the risk for grief, cultural and spiritual, verbal and nonverbal.
  • Implement the individualized POC and recommend revisions to the plan as necessary.
  • Consulting with and educating the patient and family regarding the disease process, selfcare techniques, endoflife care, nutrition, and dietary needs. Providing training to other staff as needed.
  • Initiatial appropriate preventive and rehabilitative nursing procedures.
  • Prepare clinical and progress notes that demonstrate progress toward established goals.
  • Coordinating all patient and family services and prioritization of needs with the members of the IDT.
  • Use of case management approach and refer to other services as needed.
  • Inform the physician and other personnel of changes in the patient's needs and outcomes of the intervention, while evaluating patient/family response to care.
  • Determine the scope and frequency of services based on patient/family needs.
  • Assesses the ability of the caregiver to meet the patient's immediate needs upon admission and throughout care.
  • Communicate information using current processes and technology available to the organization.
  • Perform oncall responsibilities and provide oncall service to patients/families as assigned.

Job Qualifications - Hospice:


  • Education: Graduate of an accredited Diploma, Associate, or Baccalaureate School of Nursing
  • Licensure: Current State license as a Registered Nurse, current State Drivers License.
  • Experience: Two years of experience as a Registered Nurse in a clinical care setting is required, hospice experience is preferred.

Schedule:


  • Holidays 24 hours (coordinated with 7 on/7 off)
  • Sunday 24 hours
  • Monday 5p8a
  • Tuesday 5p8a
  • Wednesday 5p8a
  • Thursday 5p8a
  • Friday 5p8a
  • Saturday 24 hours

Pay:
$75, $80,000.00 per year
