Hospice On-Call Nurse

Position Summary:


As a Hospice On-Call Nurse in Chicago, IL, you will be a pivotal member of our compassionate team, providing essential care and support to patients and families during critical moments.

Your primary responsibilities will include triaging calls, visiting declining or distressed patients, and conducting death call visits within a specified surrounding area.

You will serve as a beacon of comfort and expertise, ensuring that our patients receive the highest quality of care, dignity, and respect during their end-of-life journey.


Key Responsibilities:

Triage Calls:
Responsively assess and prioritize incoming calls from patients, families, and caregivers. Determine the urgency of each situation and provide appropriate guidance and support over the phone.

Patient Visits:
Conduct timely visits to patients who are experiencing decline or distress. Utilize your clinical expertise to assess symptoms, manage pain, and provide emotional support to patients and their loved ones.

Death Call Visits:
Respond promptly to calls notifying of a patient's passing.

Offer compassionate presence and support to the bereaved family members, providing guidance on necessary procedures and facilitating arrangements with funeral homes or other agencies as needed.

Collaboration and Communication:
Maintain clear and effective communication with interdisciplinary team members, including physicians, social workers, chaplains, and other healthcare professionals. Collaborate closely to ensure continuity of care and seamless transitions for patients and families.

Documentation and Reporting:
Accurately document all patient interactions, assessments, and interventions in electronic medical records. Provide comprehensive reports to the interdisciplinary team to facilitate continuity of care and ongoing evaluation of patient needs.

Education and Support:
Offer education and support to patients, families, and caregivers regarding end-of-life care, symptom management, and advance care planning. Empower individuals to make informed decisions and advocate for their preferences and goals of care.


Qualifications:

  • Registered Nurse (RN) licensure in the state of Illinois.
  • Minimum of [X] years of clinical experience in hospice, palliative care, or a related field.
  • Strong assessment, critical thinking, and decisionmaking skills.
  • Excellent communication and interpersonal abilities, with a compassionate and empathetic demeanor.
  • Ability to work independently and as part of a multidisciplinary team in a fastpaced environment.
  • Willingness to participate in oncall rotations, including evenings, weekends, and holidays.
  • Valid driver's license and reliable transportation for travel to patient homes.
