• 10 beds Congregate Living Health Facility with facilities in city of Moorpark, CA is looking for experienced, compassionate, hardworking LVN.
• 12 hr PM shift 1 to 2 x week.
• Participates in patient and family education, appropriately reporting the identified needs for other disciplines (OT, PT, MSW, ST, Dietician) to the registered nurse and/or Clinical Supervisor.
• Knowledge of assessment and documentation, wound care and staging, gtube placement and care, tracheostomy care and ventilator experience.
• We utilize an Interdisciplinary Treatment Team composed of physicians, nurses, case managers, social workers, physical and occupational therapists, and a registered dietitian.
• Performs direct nursing care to include PM care; skin, bowel, bladder programs, nutrition and hydration, medications and ordered treatments, transfers, NG tube and gastrostomy tube feedings; positioning and wheelchair safety.
• Current California LVN license and CPR certification (American Heart Association only), with nursing care experience in subacute, SNF or CLHF settings.
