About Us

Position Summary


Provide community and home-based case management and direct support to foster housing stability and the overall well-being of clients residing in SHC's long-term housing programs.

Intensively work with individuals and families, who are disabled and formerly homeless, on a regular basis to facilitate linkages to a range of services and resources.

Provide crucial support and coaching around psychiatric symptom management, substance abuse and a harm reduction recovery process, behavioral health management, trauma and physical ailments.

Ensure all clients are compliant with program rules, individual service plans and lease requirements.

Essential Functions,


Responsibilities:


  • Conduct comprehensive applicant interview and assessment process
  • Meet with individuals and families in their homes and in the community on a regular basis—multiple times a week
  • Provide crisis intervention, informal counseling support and advocacy services to promote health and safety
  • Provide close support around complex dual diagnosis related issues and behaviors
  • Assist with budgeting and credit repair needs; benefit attainment; access to healthcare; addressing court related issues; and medication monitoring; parenting education; connections to education, employment,
and volunteer opportunities; referrals to mainstream services; and activities related to family, social and community reintegration

  • Schedule and accompany clients to meetings and servicerelated appointments
  • Teach essential living skills, social skills, and healthy coping skills
  • Develop, monitor, and implement individual service plans (ISP)
  • Ensure housing lease compliance and maintenance of health and safety standards
  • Organize and maintain accurate and uptodate paperwork, including client files and program reports
  • Enter comprehensive individual meeting notes and assessments into online database systems (HMIS)
  • Organize curriculum for and facilitate peer support group(s)
  • Serve as liaison to landlords, property management companies and other housing agencies
  • Assist clients in maintaining apartments to meet housing quality standards by addressing sanitary and maintenance issues
Assist with client housing relocation on an as needed basis

  • Serve as intermediary and coordinator within each client's network of service providers
  • Perform outreach to community partners to promote the program and recruit applicants
  • Cultivate positive working relationships with human service and government agencies
  • Attend individual supervision, staff meetings, and communitybased meetings
  • Perform supervised urine drug screening for clients on a consistent basis
  • Provide limited after hours oncall phone availability
  • Demonstrate strong and clear communication both written and verbal
  • Abide by handbook rules, policies and code of conduct
  • Perform other related duties as assigned
Education, Abilities, Competencies and Experience Required

  • High school diploma
  • Car and valid driver's license required
  • Minimum one year relevant experience with homeless, mental health, and substance abuse services and resources
  • Flexible, reliable, motivated and possess positive attitude
  • Excellent time management skills and able to meet deadlines
  • Organized, able to multitask and followthrough with tasks to completion
  • Work well independently and as part of a team environment
Supervisory Responsibility

This position has no supervisory responsibilities.

Work Environment

This job operates in an office and professional environment as well as at client apartments and in the community. This role routinely uses standard office equipment such as computers, phones, photocopiers.

Physical Demands

This is mostly sedentary outside of travel in an office space. This would require the ability to sit at a desk, frequent walking, bend or stand as necessary.

Position Type and Expected Hours of Work


Designated days and schedule will be primarily Monday-Friday though will also be based on the needs of the operation and business.

This role does include occasional on-call availability (remote support via cell phone) and compensates accordingly.

Travel


Position requires regular travel to client apartments in Somerville, Arlington and other local communities; based on the needs of the operations and business.
