- Assist in building community relationships to obtain referrals and referral sources.
- Perform in-home assessments to determine support and client needs.
- Conduct regularly scheduled supervisory visits of clients.
- Monitor personal care and companion care services provided by direct care staff.
- Assist in educating direct care staff to ensure quality of care.
- Maintain confidentiality regarding client information and all related documentation per HIPPA guidelines
- One year of clinical nursing experience; home care experience is a plus.
- Excellent interpersonal, written, and verbal communication skills.
- Energetic team player with a positive attitude.
- Practical Nurse licensure in the state of Missouri
- Current CPR certification
- Valid Driver's License and Auto Insurance
