Summary:
The Associate Clinical Social Worker (ACSW) is responsible for welcoming, registering, and admitting new patients. The ACSW gathers information regarding the patient and creates patient files, and schedules future medical appointments. In addition, the ACSW conducts assessments and screenings for new patients.
Essential Functions:
- Interview patients and complete admission paperwork.
- Scan received documentation from patients into the system.
- Enter new patient records in the system and ensure accuracy.
- Verify patient insurance coverage and benefits.
- Responsible for scheduling patient appointments and consultations.
- Answer telephones and complete associated clerical tasks.
- Create and maintain patient records for filing.
- Responsible for assessing, screening, and diagnosing new patients.
- The Case Manager uses an integrated (medical/behavioral health) case management program and a collaborative relationshipbased approach, to assist polychronic and highrisk patients to improve health outcomes
- Provide positive, supportive and caring behavior in all communications to patients, community partners, and staff
- Outreach and marketing
- Identifies opportunities to schedule in clinic appointments to address various health needs/screenings; arranges transportation as needed to and from the clinic
- Monitor and evaluate the effectiveness of the patient's care management plan and continuously updates to ensure goals are progressing and remain pertinent, adjust goals as needed, and identify/address barriers to completing goals
- Traveling 2 times per week
- All other duties as assigned.
Education and Experience:
- Associate degree in Medical Office Management, Medical Staffing, Health Administration, or related field preferred.
- Professional certification in healthcare, preferred.
- Minimum one (1) year experience working in the medical field, preferred.
- Excellent organizational and communication skills.
- Strong customer service skills.
- Bilingual in Spanish and English, preferred.
Work Environment:
- Fastpaced environmentally controlled medical office environment
- Frequent interaction with a diverse population including team members, providers, government officials, patients, insurance companies and other members of the public.
Physical Demands:
- Frequent sitting, writing, typing, grasping and speaking.
- Occasional standing, walking, reaching, bending, stooping, lifting and carrying.
- Lifting, carrying, pushing and pulling up to 25 pounds.
- Frequent use of computer, keyboard, copy and fax machine and phone.
Pay:
From $26.00 per hour
Schedule:
- 8 hour shift
- Day shift
