Eminence Home Care is looking for a talented Caregiver/ Direct Care Worker in the Greater Hartford County, Middlesex and Tolland counties in Connecticut to join our amazing team

As a
Home Health Caregiver, you will provide direct care and support to patients in their homes or in a healthcare facility.

You will play a crucial role in ensuring that every person receives the highest level of care possible, while having an unwavering commitment to helping patients live their best lives.

You will:

Assist patients with daily living activities such as bathing, dressing, and grooming.
Monitor vital signs and record patient information.
Administer medications as directed by healthcare professionals.
Provide emotional support to patients and their families.
Collaborate with healthcare professionals to develop and implement care plans.
Ensure a safe and clean environment for patients.

You will have:

Prior experience with dementia patients and senior care
High School diploma or equivalent
Access to reliable transportation
Excellent communication and interpersonal skills
Capable of handling heavy lifting and meeting the physical demands of the role
Strong attention to detail
Compassionate and patientcentered approach
