We are looking for a Community Services Specialist, working in a
hybrid position,
40 hours/week, in our
Lewiston and satellite offices as needed.

As a team member of the Community Services Department, this position will provide support to clients within Androscoggin, Oxford and Franklin Counties.


Responsibilities
-
Provide information, advocacy, counseling and assistance to older adults and adults with disabilities and their families in identifying and accessing support services in the community:


  • Plan for long term support services:

Identify programs and services to meet the needs of older adults, persons with disabilities, and caregivers:


  • Serve as a collaborative liaison with community health and human service organizations:
-
Services are aimed at clients living within Androscoggin, Oxford and Franklin Counties

Qualifications include:

-
LSW, LSX or the ability to become a conditionally licensed social worker:


  • Bachelor's degree in Social Services, Health & Human Services or related:
-
1-3 years' experience in case management, professional social work, nursing, gerontology, health or other related human services preferred:


  • Ability to plan, organize, and coordinate multiple tasks:
-
Excellent problem solving, time management, prioritization and organization skills:


  • Effective assessment skills with the ability to successfully establish goal planning with clients:
-
Valid Driver's License

Pay:
$ $23.00 per hour
