An Eldercare Navigator acts as a liaison between the client, their support people, and healthcare providers with the goal of improving continuity of care and quality of life.

You need to be resilient, self-directed, willing to learn and grow, trustworthy, patient, and at times you will need thick skin You need sensitivity to address complex family dynamics and client preferences, and the ability to collaborate with facilities and care agencies as you advocate for clients.

Nurses, Social Workers, and licensed Physical, Occupational and Speech Therapists are ideally suited to this role, which is 20-40 hours per week, Monday-Friday.

What You Do:

Navigators travel to see clients in their homes, accompany them to appointments, and provide care coordination.

Your approach is holistic and client-centered with a focus on managing needs in crisis and creating proactive plans which anticipate future needs.

This is done through assessment, planning, problem-solving, monitoring, education, and advocacy.

You will need to train with us at our office in Somerville, accompanying Navigators to client homes to understand the culture and values essential to being a Nightingale Navigator.

Orientation is Monday-Friday 9-5. Once you see clients independently, your schedule is more flexible, meeting clients at times that work for you together.

When you assume the role of Primary Navigator for specific clients, you are available to them during regular business hours, but always have the support of our team, and a back-up colleague who knows your clients and can assist when you are not available.
