    Description

    Essential Job Responsibilities

    Care Coordination and Caseload Management

    Provides care coordination to Miami/Fort Lauderdale area minorities using program methodology.


    Identifies and serves clients who are HIV+, newly Diagnosed or lost to care for at least 6 months with multiple complex problems and primary medical care needs who are having or likely to have difficulty initiating or sustaining that care.

    Introduces and describes the goals and objectives of the program to clients.

    Encourages clinic contact and emphasizes success through mobilizing strengths and abilities in the client's preferred language.

    Reviews with each client his or her thoughts upon receiving news of HIV infection.


    Conducts five structured care coordination sessions with each client who does not access care (fewer if the client does access care).

    Identifies and emphasizes the client's strengths, abilities, and skills.

    Addresses client's personal needs and barriers to health care.

    Solicits client concerns and questions.

    Summarizes major points of each session with a client.

    Prepares for the next contact with each client.

    Determines role of important relationships with client's medical care attendance.

    Initiates and finalizes the disengagement process with the client.


    Provides coordination of services with follow-up for each client and coordinates service provision with area services to ensure the client is appropriately linked to care system.


    Responsible for recruiting and linking at least 13 clients monthly into the ARTAS program.

    Service Planning and Documentation

    Develops a recruitment and engagement strategy according to program requirements.

    Provides a monthly report of clients accessing ARTAS and medical care to Health Promotion, based on referrals made from the previous months

    Ensures all documentation is Timely, Accurate, Legible, and Clear.

    Prepares necessary program reports and records as requested by supervisor.


    Maintains treatment plans, progress notes, and progress reviews in client records as specified in funding sources' guidelines and performance standards.


    Administration, Compliance and QA

    Inputs client information using specific software as required.

    Supports billing and budget activities as required.

    Attends appropriate training, case conferences, re-entry/health fairs, assigned committees, and skill-building activities.

    Adheres to agency procedures and protocols in the provision of effective delivery of program services.

    Participates in audits, site visits, and meetings as required by supervisor.

    Participates in Quality Assurance activities as required.

    Contact Responsibility:

    The responsibility for internal and external contacts is frequent and important.


    Physical Requirements:

    This work requires the following physical activities: constant talking in person, talking on the phone, hearing/ visual acuity. Frequent walking, sitting, and bending. Occasional standing, driving, and climbing may be required. Work is performed in the office, or where clients or potential clients are located.

    Participates in health center developmental activities as requested.

    Other duties as assigned.
