    Description

    This is an exempt position with the Pikes Peak Area Council of Governments Area Agency on Aging (PPACG AAA). This position serves as the Program Coordinator for the PPACG AAA Family Caregiver Support Center, and is a Care Planner for informal caregivers who contact the Center for assistance. As the Program Coordinator, this individual is responsible for administration of the Caregiver Support Program under the guidance of the PPACG AAA Director, for participating in collaborations of community organizations that have services to benefit the caregivers, and for providing information and assistance to caregivers to help them access those services. As the Center's Care Planner, the individual assists informal caregivers to manage their care giving responsibilities by developing a plan to support them in caring for an elderly spouse, parent, family member, or friend.

    Duties include, but are not limited to:

    • Collaborate with community organizations that have services for informal caregivers to assist them in managing their informal care giving responsibilities for the elderly.
    • Oversee operations of the PPACG AAA Family Caregiver Support Center with the goal of providing information, assistance, and access to services.
    • Manage the Center's marketing campaign to raise public awareness about the Caregiver Support Program and assist the effort as appropriate under the PPACG AAA Director's supervision.
    • Develop individual care plans for caregivers who contact the Center for information and services.
    • Refer individual caregivers to the proper services and assist them in accessing those services.
    • Identify "gaps" in services to caregivers and provide guidance in encouraging development of those services in the community.
    • Work with the PPACG Website Coordinator to manage the portion of the PPACG website dealing with the Family Caregiver Support Center.
    • Speak to various groups and organizations to educate, advocate for, and increase awareness of caregiver needs and to promote services provided by the Caregiver Support Center.
    • Prepare monthly performance reports for PPACG AAA on persons served, units of service provided, type of services, and appropriate data on these services as needed by the PPACG AAA and/or the State Unit on Aging.
    • Administer the FCSC Respite Voucher Program.
    • Analyze need for additional services for caregivers and implement these services with the approval of the PPACG AAA Director, as appropriate.
    • Participate in community senior events, as needed.
    • Provide input on the content of the Yellow Book and the Senior Information and Assistance Center (SIAC) database and other SIAC resource materials as they relate to informal caregivers.
    • Provide input to the Associate Director for the tracking of Caregiver Respite vouchers issued by the PPACG AAA and the input of data for reporting activities into the reporting system required by the State of Colorado.
    • Serve as back-up for the SIAC Case Manager.
    • Perform other work as requested by the PPACG AAA Director.

    Requirements

    Required Education and Experience

    1. Bachelor's Degree in a human services curriculum plus a minimum of four years of progressively responsible experience with an agency serving human resource needs, providing counseling or case management. A Bachelor's Degree in Social Work is preferred, or

    2. Master's Degree in a human services curriculum years of progressively responsible experience with an agency serving human service needs, providing counseling or case management. A Master's Degree in Social Work is preferred, or

    3. Associate's degree in a human services curriculum plus a minimum of 6 years of progressively responsible experience with an agency serving human service needs, providing counseling or case management, or

    4. May have commensurate work experience of at least 8 years in lieu of a degree.

    •The ideal candidate must possess a strong understanding of available resources for senior citizens and their caregivers within the community.

    •Knowledge of local community service agencies is important.

    •Superior verbal and written communication skills are essential.

    •Basic computer skills including Microsoft Word and Excel. Database experience is helpful.
