Job Summary:
We are seeking a dedicated and compassionate Case Manager to join our team. As a Case Manager, you will be responsible for providing support and guidance to individuals and families in need. Your role will involve assessing client needs, developing care plans, coordinating services, and advocating for clients. Duties:

• Conduct comprehensive assessments of client needs and develop individualized care plans
• Coordinate and monitor services provided to clients, ensuring they are receiving appropriate support
• Collaborate with other professionals, such as social workers and healthcare providers, to ensure holistic care for clients
• Provide crisis intervention and support during challenging situations
• Advocate for clients' rights and access to resources
• Monitor client progress and make necessary adjustments to care plans as needed
• Maintain accurate and uptodate documentation of client interactions and services provided

Qualifications:

• Bachelor's degree in social work or a related field
• Previous experience in case management or a related field
• Knowledge of child welfare systems and services
• Familiarity with behavioral health principles and interventions
• Experience in mentoring individuals or families facing challenges
• Understanding of social work ethics and standards of practice
• Knowledge of crisis intervention techniques
• Familiarity with programs such as SNAP
• Understanding of workforce development resources and strategies

$41, $44,000.00 per year
