- Conducting triage and intake interviews with clients and potential referral sources in person.
- Providing crisis stabilization services, counseling, education, community resource referrals, and social services interventions for youth and families.
- Completing counseling and social work assessments, mental status exams, and social histories.
- Coordinating and monitoring various services in the community, such as mental health, medical, educational, vocational, and social services.
- Facilitating group therapy sessions on relevant topics based on patient care needs.
- Assisting in scheduling group therapies and modalities within the CSU team.
- Assessing, determining suitable treatments, and developing treatment plans.
- Linking clients to case management and other essential services.
- Maintaining accurate and comprehensive clinical records.
- Collaborating with hospital staff to facilitate the step-down of patients from inpatient care to CSU.
- Establishing and implementing prevention strategies.
- Conducting crisis follow-up on specific issues within defined time frames.
- Ensuring timely and detailed documentation of services to meet industry standards.
- Providing brief, solution-focused counseling on specific issues.
- Collecting necessary client assessment and demographic information.
- Accurately documenting service provision for billing purposes.
- Providing critical incident intervention when traumatic events occur.
- Maintaining certifications in CPR, First Aid, and Crisis Intervention.
- Performing additional duties as required.
- Licensure: LSW/LPC; LISW/LPCC
- Minimum 2 years of crisis intervention experience
- Valid Ohio driver's license
- Eligibility to drive company vehicles with insurance coverage
- Bachelor's degree required; Master's degree preferred
