- Perform comprehensive nursing assessments and collaborate closely with the nursing team, alongside other professionals, patients, and families to create effective care plans.
- Set achievable short and long-term patient goals with necessary interventions to reduce rehospitalizations, continuously assessing outcomes.
- Administer medications and treatments as prescribed by physicians.
- Assign and oversee patient care duties to staff, ensuring proper implementation of tasks and interventions.
- Facilitate the seamless flow of patient information between assigned staff and shifts.
