Overview:


Assists and supports patients, families, community members, and health care providers with accessing and utilizing resources, information, education inclusive of standardized self-management psychoeducation, and other supportive services, interventions and community trainings related to Social Determinants of Health (SDoH).


Responsibilities:


  • Reaches out to patients identified by HOBSCOTCH Institute and epilepsy clinic intake forms and SDoH screenings.
  • Provides assistance in the identification and referral to selfmanagement and community resources for the health care team and patients/families. Meets virtually with patient to assess the need for services. Provides patient information and support concerning available resources. Assists patient in obtaining appropriate resources. Maintains expertise in resources for addressing SDoH needs.
  • Updates the Community Resource Directory, utilizes RedCap database for tracking of interactions, assists with community engagement studios
  • Participates in training to be knowledgeable about evidence based selfmanagement curriculum and assists with review of materials as part of the HOBSCOTCH Institute team.
  • Identifies and contributes to the development of systems and resources to assist patients, families and health professionals.
  • Performs other duties as required or assigned.

Qualifications:

  • High School graduate or equivalent required.
  • Must successfully complete Community Health Worker training provided.

Required Licensure/Certifications:

  • Valid driver's license with a clean driving record required in accordance with D-H's Driver Disqualifications Guidelines.
