Summary
We currently have a full-time opening for an Outreach and Enrollment /SCO Coordinator for our Gloucester Family Health Centers.

The Outreach and Enrollment /SCO Coordinator, reporting to the Manager of Patient Services, will work full-time to conduct culturally appropriate outreach to a broad array of organizations in Gloucester and the North Shore community.

These include; faith-based organizations, food pantries, shelters, Boards/Departments of Public Health and Education, Chamber of Commerce, etc.

The outreach will include education on the following:

the process for determining eligibility for public and/or subsidized insurance products; the impact of the ACA 2014 Medicaid expansion on low-income MA residents; how to use the MA health insurance exchange.

In addition, the Coordinator will enroll consumers in and around the Salem community into public and/or subsidized insurance programs on-site at health fairs, street fairs, festivals, shelters, drop-in centers, food pantries, churches, etc.

The Coordinator will also assess the eligibility of existing and future NSCH patients in insurance programs and enroll them as appropriate.

The Coordinator will collaborate with Site Managers and Patient Service Representatives. The Medical Support Coordinator will serve as an onsite liaison between CCA, SCO program and NSCH. He/She will serve as a resource for all providers.


As a team member you will work with staff who are racially, ethnically and culturally diverse, so being culturally competent is required.

A sense of humor is also very important


Responsibilities

  • The Outreach and Enrollment / SCO Coordinator will work with the Manager of Patient Services to develop a detailed work plan with outreach and enrollment targets for Salem and the North Shore community
  • She/he will attend all required state and federal enrollment trainings, as needed
  • He/she will track progress of attainment of targets for his/her site
  • She/he will identify and reach out to community organizations to schedule presentations and/or outreach events
  • He/she will distribute high quality outreach materials to residents of Salem and the North Shore community
  • She/he will identify and collaborate with other O&E program staff at NSCH He/she will attend evening and weekend community events (health and street fairs, festivals, other events), as needed, to educate and enroll consumers in insurance programs on site.
  • She/he will work with existing and future NSCH patients to assess eligibility and enroll in insurance programs
  • He/she will collaborate with NSCH Patient Service Representatives to ensure that the eligibility and enrollment needs of NSCH's patients are met with a demonstrated commitment to high quality customer service
  • Act as an Element Care SCO liaison and resource on site for all NSCH sites
  • Assist SCO patients in making appointments at NSCH sites
  • Track Element Care SCO members specialist referrals and results of specialist appointments and share information with all NSCH providers
  • Communicate with SCO team about medical updates and /or member needs/ request
  • Maintains member confidentiality at all times and translating, if needed.
  • Run quarterly reports for potential SCO patients, and reach out to them

Requirements:


QUALIFICATIONS

  • Experience in a community health center setting and/or related experience in health/human services and/or social services agency
  • Demonstrated strong communication skills with individuals and/or groups
  • Fluency in English AND Spanish or Portuguese
  • Valid MA driver's license
  • Maintains strict adherence to the NSCH Confidentiality policy
  • Incorporates NSCHI values, principles and mission statement into daily activities
  • Complies with all NSC
