With 118 beds and more than 400 physicians on its medical staff, Pali Momi Medical Center offers a full range of services for the communities of Central and West O'ahu.
The hospital has delivered many medical firsts for the community, including Central and West O'ahu's only interventional cardiac catheterization unit to detect and treat heart disease, a fully integrated minimally invasive surgical suite, a comprehensive women's center, CT scan and MRI services, and the state's first retina center.
Pali Momi is the only facility in Hawai'i that uses a team triage approach in its emergency room, where patients are promptly evaluated by an emergency physician.
The Case Management team puts great emphasis on helping each patient transition their health care treatment with an appropriate set of support tools and information.
Our caring, professional staff provides timely medical and discharge management to meet clinical and service goals, and to provide an effective, appropriate continuum of care.
We want to ensure that our patients receive the attention they need beyond the hospital walls, especially when they require care from several different organizations and other combinations of medical, financial or social interventions.
As the Registered Nurse Case Manager, you would ensure the considerate and confidential management of unique and high-risk patient cases by performing case management through direct referrals and reviews of medical records.
This would include determining the appropriateness and medical necessity of admissions, continued hospital stay, and the use of ancillary services.
You would also perform treatment plan assessments, discharge needs interviews, planning/coordination for high risk cases, and participate in organizational quality improvement activities.
We are looking for an individual who is perceptive to patient needs, with outstanding organizational and communication skills and a commitment to delivering the highest quality health care to Hawai'i's people.

Minimum Qualifications:
Current Hawai'i Registered Nurse license. Bachelor's degree and/or equivalent combination of education, training, and work experience. Three (3) years recent acute care nursing experience in Pediatrics, OB/GYN, or Adults, or related experience.
Preferred Qualifications:
Current Case Management or Utilization Management certification. Case Management experience and knowledge of Interqual.
