Davis Place, a beautiful 5-star, 190 bed, privately owned skilled nursing facility in Danielson CT, is seeking an experienced
Social Worker to join their team of highly skilled professionals. Davis Place is a newly renovated facility with a great managerial support structure and is well known for their outstanding patient care and legendary customer service.
Responsibilities include but are not limited to:
- Develop preliminary/comprehensive assessments for each resident.
- Report any issues/ changes to the Director of Nursing Services
- Encourage the resident/family to participate in the developmental process and review of resident's plan of care.
- Develop a written plan of care for every resident that identifies all social problems and needs of the resident, followed by setting goals to be accomplished for each problem/need identified.
- Ensure that all social services components of the MDS reports are completed and signed in the most efficient manner.
- Review nurse's documentation to determine if the care plan in place is being followed properly.

Experience:
- LTC: 1 year (required)
License/Certification:
- BSW (required)
