SUMMARY:
Under direct supervision of Health Information Supervisor, maintains complete medical records according to established procedures.
Files and retrieves patient records, scans/indexes external documents into electronic chart; and assist in the distribution of medical records mail.
Provides support assistance to HIT technologies.-

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Record Management:
- Organizing and maintaining patient records, including electronic health records (EHRs) paper files on premises and offsite storage
- Ensuring accuracy, completeness, and confidentiality of medical information. Scans/indexes external documents appropriately into patient chart
Release of Information:
- Responding to requests for medical records from patients, healthcare providers, insurances companies, and legal entities
- Verifying authorization and releasing records in compliance with privacy laws (e.g., HIPPA)
Auditing and Quality Control:
- Regularly review incoming requests from external institutions for accuracy, and compliance.
- Identifying and correcting errors or discrepancies.
Collaboration:
- Working closely with healthcare professionals, administrative staff, and billing department.
- Assisting with medical records audits and ensuring proper documentation.
Archiving and detention:
- Managing the retention and disposal of records according to legal requirements.
- Archiving inactive records for longterm storage.
Training and Education:
- Training new staff on recordkeeping procedures and software
- Will serve as support for multiple HIT systems up to and including the patient portal
- Manage medical record portal desktop and respond to inquiries.
- Assist in preloading vaccine records.
- Staying unformed about changes in regulations and best practices
- Performs other related duties as assigned.
This position requires compliance with all of Alivio's written standards, including its Standards of Conduct, Joint Commission standards, all policies and procedures and Corporate Compliance requirements.
SUPERVISORY RESPONSIBILITIES:
Manages and supervises staff listed below, including hiring, training, evaluating and managing performance to meet departmental efficiency and performance measures in keeping with the Union Collective Bargaining Agreement (CBA), Personnel Policies and Procedures and prevailing employment laws as appropriate.

WORKING CONDITIONS:
OSHA Category - Involves no regular exposure to blood, body fluids, or tissues, and tasks that involve exposure to blood, body fluids, or tissues are not a condition of employment.
QUALIFICATIONS:
Education:
- High School graduate or GED, some college preferred
Certification of Licensure:
- N/A
Experience:
- Six months clerical experience; experience with medical terminology preferred and basic computer data entry
Special Training:
- Computer systems, filing systems
Demonstrated Competencies:
- Ability to maintain alphabetical and numerical filing systems; ability to use computer systems and office equipment; ability to learn scanning/indexing processes; ability to complete routine paperwork; ability to maintain confidentiality of records and information; ability to maintain quality, safety, and/or infection control standards; ability to follow routine verbal or written instructions
