The Grand Rehabilitation and Nursing Center at
Pawling, located in Pawling, NY, is seeking LPNs. Postion can be full-time, part-time or per diem. We have availabilities during all shifts.

LPN Responsibilities:

Implement and coordinate care delivery in collaboration with physicians and care associates.
Conduct health and wellness checks for residents regularly.
Assist in developing and updating individual service plans for residents.
Maintain open communication with residents, families, and physicians regarding their needs.
Utilize the nursing process (assessment, planning, implementation, evaluation) for optimal resident care.

LPN Requirements:

Current and valid NYS LPN License in good standing.
Ability to work effectively in a team environment.
Prior experience in a Nursing Home or Hospital preferred but not required
