Summary Job Description:


Please note:
this position requires a valid and unrestricted New York State driver's license and comfortability with transporting children.


Principle Responsibilities:


  • Facilitates the exchange of health information and documentation of care received by the child in the community including attending any communitybased care
  • Transport patients inside and outside of the facility to designated locations in a timely manner
  • Coordinates Medicaid services that the child receives in the community and transitions the child to home or communitybased living with outpatient treatment (e.g., individual and family therapy) as part of discharge planning
  • If the child attends school in the community, care at school and delivered through the provider must be coordinated
  • Assists with obtaining outside provider documentation
  • Assists Medical and Mental Health coordinator with scanning and filing of documentation.
  • Data entry into the intranet for psychiatric and psychological evaluation diagnoses
  • Coordinates with Nurse Case Manager regarding Health Education and Substance Abuse Requests
  • Answers and transfers phone calls to appropriate personnel and takes messages
  • Schedules Mental Health and Medical appointments as assigned
  • Assists with calling pharmacies and all other appropriate parties for Medication Authorization Requests
  • Maintains the MH Appointment Scheduler by marking appointments as held, cancelled, or kept and rescheduling medication reviews if missed or cancelled
  • Updates case planner Unit Meeting Minutes with recommendations for client and paperwork needed for auditing purposes
  • Additional tasks as assigned by Nurse Supervisor

  • Receive training that is commensurate with the intensity, scope and duration of the needs of the children served.

Training must include the following:
Mandated reporter, Risk and safety, Trauma informed care, Family engagement, Basic child development

  • Current certification by the American Red Cross (or an equivalent) in First Aid and cardiopulmonary resuscitation (or willingness to be certified before starting)
  • Additional training for health care needs of children which are age and need appropriate

Minimum Requirements:


  • Be at least 21 years of age
  • High school diploma or equivalent, BA or MA preferred
  • A valid NYS driver's license is required

Travel Requirements:

This position is located in Brooklyn and transporting youth to different facilities.
